Please fill out this form as thoroughly as possible to be considered as a new client. Don’t worry, it’s not an application and it’s not a test! It’s just a way to make my job a lot easier by consolidating a majority of the information I need from new clients all in one place I can reference later if needed.

My schedule is very limited at this time, filling out this form does not guarantee booking. You’re always welcome to fill out the form again if you don’t hear back from me after a few weeks.

Masks are required at wild pansy studios and must be worn through the entire appointment. We have some available if you need.
